This Taranaki education series is designed to strengthen collaboration between general practice teams and Extended Care Team specialists. In this session learn about the role and scope of social work in primary care, when to engage a social worker, and best practice processes for accessing support in areas including housing, financial hardship, health-related assessments, and hoarding and squalor.

    About this event

    This Taranaki education series is designed to strengthen collaboration between general practice teams and the Pinnacle Extended Care Team. Each session provides practical insights into specialist roles, referral processes, and best practice approaches to patient care.

    All presentations include clear guidance on referral processes and expected correspondence for at-risk patients, alongside practical resources aligned with guidelines such as HealthPathways, BPACnz, and Ministry of Health publications.

    Sessions incorporate reflective practice through case discussions and real examples, ensuring content is relevant and applicable to your daily practice. All content is peer-reviewed before delivery.
